2. 2022 Elm Grove Beautification Committee Shrub Order Form

   The Beautification Committee will be selling bare root 18-24 inch locally grown, native shrubs. We have selected 6 varieties for you to choose from. You may fill out the form below and drop off or mail your check to Village Hall to be added to the 2022 Shrub sale. Your order will be limited to a maximum of 5 of each variety. Pickup will be during the Earth/Arbor Day celebration on April 23, 2022 - the perfect time for planting!

    This form holds your place, but does NOT ORDER YOUR SHRUBS. Orders are placed first come, first served based on receiving payment. We currently accept check, cash or credit card. Please make your check payable to the Village of Elm Grove, 13600 Juneau Blvd., Elm Grove, WI 53122. Checks may also be placed in the after hours drop box at Village Hall. Cash accepted in-person only, Monday - Friday 8:00am to 4:30pm at Village Hall. For credit card payment, please use the link provided on the "submission" screen after you've submitted your order.
